1. Passport validity and blank pages

• A passport valid for at least six months from your planned arrival date in Indonesia
• At least one blank page for stamps where required
• A clear scan or photo of the bio-data page (name, photo, passport number, expiry) if applying online

• Name spelling that does not match your ticket or application form

2. Passport-style photo

Online applications usually need a recent passport-style colour photo:
